| To view, fill out online, and print these forms you will need Adobe Acrobat® Reader®.
If the form you want is not available here, please call the CKC Order Desk at 1-800-250-8040.
- All Registration forms must be signed in ink and returned by mail.
- All Shows & Trials forms may be returned by mail or faxed to the CKC at 416.675.6506.
If the PDF files do not open try saving the file to your PC using the instructions below:
- Saving PDF from Netscape, Firefox, and Internet Explorer
Right mouse click the link and select "Save Link As." or for Internet Explorer "Save
Target As."
Follow prompts to save the file to your location, taking care to recognize where
you saved the PDF file. You may then open it within Acrobat Reader.
- Saving PDF files to Macintosh
Either control+click or click and
hold the mouse button down on the link until the menu pops up. Select the item
from the menu that indicates it will "save" or "download" the file to the disk.
Select the file type before you click OK (the file type should be PDF or source,
depending on your browser).
